Why Knee and Back Pain Often Starts with Your Shoes

If your knees or lower back ache and no amount of stretching seems to fix them, look down. The problem often begins with your feet, and with the shoes you put them in. I am Geon Seo, owner of JGP USA on Oracle Road in Tucson. I spent my earlier career as a medical device engineer, and I would like to share how an engineer looks at a pair of shoes.
Think about the last time you bought shoes. Did you choose them because they looked good, or because they felt soft the moment you slipped them on? When we buy a car, we inspect the engine. When it comes to our feet, the foundation that carries our entire body weight, we are surprisingly forgiving.
Your Feet Are Precision Instruments
Each foot is built from 26 bones and a network of joints, and together they act as precision sensors. Every second you stand, they send a stream of signals to your brain to keep you balanced, and every step loads them with more than your body weight. In engineering terms, your feet are the foundation of the whole structure.
A building with a shifting foundation does not show the damage in the basement. The cracks appear upstairs, in the walls and the ceiling. Your body works the same way.
How the Wrong Shoes Start a Chain Reaction
A shoe that is nothing but soft does not hold the frame of your foot. Without that support, the arch begins to collapse, and the work your foot should be doing is passed up the chain. Your calves take some of it. Your knees take more. Your lower spine takes what is left.
That is why the true culprit behind mystery joint pain is so often a neglected foot. The knee that hurts is frequently the victim, not the cause.
What a Supportive Shoe Should Do
- Hold your heel steady instead of letting it roll inward or outward
- Support the arch so it cannot flatten step after step
- Stay firm through the middle of the sole rather than folding like a sponge
- Leave your toes room to spread and grip
You can test much of this in the store. Press the heel cup and see if it keeps its shape. Try to twist the sole from both ends. A shoe that folds in half is a shoe that will let your arch do the folding later. Soft cushioning feels pleasant for a minute. Structure is what your joints thank you for after a full day on your feet.
In a coming post, I will explain how we think about this the way a mechanic thinks about wheel alignment on a car, and what the wear pattern on your soles says about the way you walk.
